Description

The service is for the delivery of a Hospital to Home service to adults in Leicestershire who are being discharged from hospital. The primary target group will be adults aged 55 years and over. It is expected that the majority of the service will be delivered to individuals over 55, but it will be offered to individuals under 55 in appropriate circumstances.
The service will be to assist eligible adults across Leicestershire, who need low level, and temporary support that reduces hospital readmission and promotes health and well being.
There are two lots, and tenders may be submitted for either lot or both lots.
LOT 1: Relates to County Community Hospitals
LOT 2: Relates to the University Hospitals of Leicester

Option to extend	Recurrence of the contracts will depend upon the continued availability of funding for the services. There is an option to extend for up to two further years (in increments of one year) following the end date shown below.
Estimated ValueText Description	The figures shown above are the year-one total value for both lots. The actual value for lot 1 is £87.6k and for lot 2 £100k. Values beyond year one will be dependent upon the levels of continued funding.
